CJE Waiver Form

LIABILITY RELEASE

This is an important legal document and must be executed in order to participate in any Compton Junior Equestrians or Compton Cowboys programs or activities (hereafter referred to as "CJE Programs").

Please read this carefully. Fill in the information, initial, check, and/or sign this release in ink, where indicated. Must be 18 or older to sign. If you are a minor, you must have your parent/guardian complete it on your behalf.

I, the undersigned participant (if 18 years of age or older) or parent or legal guardian of the participant named above (either of the above shall hereafter be referred to as (the "Participant/Student"), hereby request permission for the above-named Participant/Student to participate in any CJE Programs, either on-site or off-site, and hereby agrees to the following terms & conditions:

1. I understand that there are risks and hazards inherent in handling and riding horses and in participating with CJE staff, horses, volunteers, that the behavior of horses cannot be predicted or always controlled, and that participating in a CJE Programs carries the risk of serious injury or death to me.
2. To help minimize the risk associated in my participation with CJE Programs, I agree to wear a helmet at all times while mounted and to otherwise fully comply with instructions of CJE Staff and volunteers.
3. I understand that riding horses is a strenuous and physically demanding sport. I represent and warrant that the Participant is physically and mentally able to participate with CJE Programs in and these activities.
4. I acknowledge that use of the CJE facilities and participating at CJE Programs is completely voluntary and that Participant/Student does so at his/her own risk and assumes all risks associated with such participation.
5. In consideration of the participant being permitted to participate at CJE Programs, the Participant and my /his/her beneficiaries, administrators, executors and assignees, do hereby, without reservations, waive, release, acquit and forever discharge CJE and all CJE agents, the owner of any horses on loan to the CJE, their representatives, officers, agents, sponsors, employees, and volunteers, from any and all liability, from suits, claims, demands or assertions of liability of any nature whatsoever, for personal injuries, property damage, injury to personal or incorporeal interest, or other like damages from or otherwise connected with the Participant's/Students participation in said CJE Programs; and do hereby covenant that no action at law or suit shall ever be brought against the aforementioned representatives, officers, directors, agents, sponsors, volunteers or employees of CJE on account of any matter.
6. I hereby irrevocably grant to Richland Boys Entertainment, LLC ("RBE"), Star Pen Entertainment, Inc. ("Star Pen"), Compton Junior Equestrians ("CJE"), and Randall Louis Hook p/k/a Randy Savvy (collectively "RS") (RBE, CJE, Star Pen, and RS (collectively "CC/CJE Management"), and their approved assignees, the right to include my child's image, likeness, actions, and statements in any live or recorded audio, video, film, webcast, stream, or other transmission, exhibition, simulcast, or reproduction made at the CJE Campus, before during or after a CJE Event, in any medium or context for any purpose, including commercial or promotional purposes, without further authorization, compensation, or otherwise in perpetuity.
7. I have carefully read and fully understand all the above information, declarations, requirements, warnings and precautions and I am fully aware of the risk factors of participating in CJE Programs and sign this authorization voluntarily.
